 The Seward Community Library & Museum is a U.S. Passport Acceptance Facility, which allows us to accept U.S. passport applications under the guidelines of, and on behalf of, the U.S. Department of State.

We are not part of the application process beyond this function and we have limited availability of Passport Acceptance Agents.


Plan Ahead

Plan far in advance to your need.  It is recommended that applications be submitted at least 3 months prior to need.  We have limited staff availability for appointments due to the many other duties that library staff are required to fulfill. 

  • Routine Service:  16 weeks, door-to-door, from date of application.
  • Expedited service:  12 weeks, door-to-door, from date of application for an additional $60.

Renew by Mail

Visit Renew my Passport to find out if you are eligible to renew your passport by mail using the DS-82 U.S. Passport Renewal Application for Eligible Individuals form, available online and at the library front desk.


Schedule an Appointment

Passport appointments are scheduled only after all parts of the application have been fully prepared.  Use only black ink.  Do not use white out.  If you make a mistake, start a new form. Do not attach the photo to the application.  Do not sign the application.

Call (907) 224-4082 to review your application(s) for completeness and schedule an appointment with a passport agent.


Cancellations

 The following will result in a cancellation of the appointment:

  • 10 minutes or later.
  • Incomplete application.
  • Application not completed in black ink.
  • Signed application.
  • Missing documents or information.
  • Child and all parents not present for a 16 and under passport.

Appointment Checklist

Prepare for your appointment using the checklist below or download our Passport Application Checklist (pdf).

  • 1 acceptable passport photo. (do not wear glasses)
  • 1 certified birth certificate or naturalization certificate.  (not a photo copy)
    • Must have an embossed seal, signed by authority and include date filed, date of birth, full name and parent's name.
    • If you don't have a certified certificate, ask us where to get one before making an appointment.
  • 1 valid driver's license and/or other form of government issued photo I.D.
    • Out-of-state IDs require an additional source of identification.
  • 1 passport application completed in black ink, not signed, no whiteout, printed one-sided.
  • 1 check or money order (no cash or cards) payable to:  U.S. Department of State
    • $130 if you are 16 or older for a Passport Book.
    • $100 if you are under 16 for a Passport Book.
    • $30 if you are 16 or older for a Passport Card. (NOT valid for air travel)
    • $15 if you are under 16 for a Passport Card. (NOT valid for air travel)
    • *Expedited Service is an additional $60.00 per application. (Passport Book only)
    • *Overnight Delivery Return Fee is an additional $19.53 per application.
  • 1 separate check or cash for $35 per application payable to Seward Community Library & Museum.
    • No credit or debit card payment option.
